Add an issue reporter which provides a more guided experience for reporting problems that is able to collect data about the user's VSCode instance. It should

  • be accessible from the Help menu and command palette
  • launch in a separate window
  • collect the same information as the --status command, system info, workspace info, and processes
  • send different data based on what type of issue is being reported
  • search for duplicate issues on GitHub after user has typed a title
  • allow user to remove or scrub info being sent
  • integrate with upload service so users can upload logs
  • on submit, preview issue on GitHub
